An individual women’s questionnaire collected information from women ages 15-49 on topics such as reproductive history, contraceptive use, child health, marriage, sexual activity, immunization, and fertility preferences. An individual men’s questionnaire collected information from men ages 15-64 on many of the same topics as the women’s questionnaire.
