This health facility assessment survey was developed by the Japanese International Cooperation Agency (JICA) in order to provide information regarding health care system assets and their functionality. Topics covered include human resources, building infrastructure, health care service availability and delivery, and utilities. The HFC methodology was designed to include all public and semi-public health service providers as well as major private providers. The main data collection tools included a health service module, building assessment module, utility module, medical equipment module, and human resources data form.
