Conducted by the National Center for Health Statistics of the Centers for Disease Control, this is a national survey with questionnaires in three parts for nursing home facilities, staff, residents, and discharged patients. Staff questions include education level, employment benefits, working conditions, the presence of unions, recruitment and retention, job satisfaction, specific duties, continuing education and professional development opportunities, and salary.  Facility questions ask about number of residents, admissions, beds, waiting lists, ownership, rates charged patients, accreditation status and use of electronic medical records.
