Summary 

The National Health Interview Survey (NHIS) is a primary source of information on the health of the population of the United States. The NHIS is a cross-sectional household interview survey that is fielded continuously throughout the year. The interviewed sample for the 2004 NHIS consisted of 36,579 households, yielding 94,460 persons in 37,466 families.

The 2004 NHIS covered a wide variety of topics, such as family composition, socio-demographic characteristics, injuries, health care access and utilization, health insurance, income and assets, health conditions, health behaviors, physical activity, and immunization. The survey included supplement sections on children's mental health and adult immunization.
