The survey data provides estimates for all six districts of the country, as well as urban and rural areas separately. The survey used three questionnaires: A household questionnaire collected basic demographic information on household members and covered topics such as education, water supply, sanitation, disability, and iodine supplements. A questionnaire for individual women ages 15-49 collected information relating to child mortality, tetanus toxoid vaccination, maternal health, contraceptive use, sexual behavior, and health literacy. A questionnaire that collected data on children under-five covered topics such as child development, vitamin A supplements, breastfeedings, immunization and anthropometry.
