Four questionnaires were used for this survey - the household questionnaire, woman's questionnaire, biomarker questionnaire, and two verbal autopsy questionnaires. There was also a self-administered questionnaire completed by the survey's fieldworkers. 30,018 households were successfully interviewed and of those households, 30,078 women completed individual interviews. 19,987 women completed the full questionnaire.
