Data collection occurred at Comilla Medical College Hospital, Chandpur District Hospital, all upazila health complexes in the Chandpur district, and some private hospitals in the Chandpur district. Once the death data were collected from hospitals, verbal autopsy interviews were conducted in households. Questionnaires for these interviews covered several topics, including diseases and symptoms predating death, housing conditions, health care use, and access to health facilities.
