The medical questionnaire forms were updated from those used in the previous surveys to provide improved surveillance of new procedures and the comparison of conventional and digital methods, while still allowing direct comparison with previous survey results. The 2006 report contains summary data from the 2001, 2004 and 2006 surveys.
