The report, Surveillance for Certain Health Behaviors and Conditions Among States and Selected Local Areas --- Behavioral Risk Factor Surveillance System, United States 2004, provides results from the 2009 Behavioral Risk Factor Surveillance System (BRFSS) survey. BRFSS began in 1984 and gathers information on health risk behaviors, preventive health practices, and health care access primarily related to chronic disease and injury. BRFSS is conducted in all 50 states, the District of Columbia, Puerto Rico, the U.S. Virgin Islands, and Guam. In 2004, 50 states, Washington DC, Puerto Rico, and the Virgin Islands conducted the BRFSS; a list of data available by state and year is available on the BRFSS site.
