Summary 

The 1995-1997 Disability Followback Survey (DFS) was a survey administered to individuals who were identified as disabled in the the 1995 National Health Interview Survey. As a followback to the 1995 NHIS, the 1995-1997 DFS utilized the following questionnaires: children with disabilities, adults with disabilities, and individuals 18 and over who reported having poliomyelitis. The 1995-1997 DFS covered topics such as housing, transportation, health insurance, use of health care services, vocational rehabilitation, work history, health opinions and behaviors, and community services for seniors. Together, the 1994-1997 DFS and the 1995-1997 DFS constituted Phase 2 of the National Health Interview Survey on Disability.
