Summary 

This extensive base-line survey includes 14 separate questionnaires designed for heads of household interviews, children, schools and their staff, and health institutions and their staff. Of significant interest are the questionnaires for women in reproductive ages 14-49, which collect complete birth histories, and information on inpatient and outpatient health care use by children and adults. The healthcare facilities portion asks about type and condition of facility, what conditions were treated there, number and type of personnel, educational levels of personnel, costs of providing the service and amount charged to patients, medicine and medical supply use, and waste of unused resources.
