The template survey includes 10 core and four optional modules. Topics include household characteristics, partnerships, fertility, and use of contraception. Each country developed a unique questionnaire derived from the standardized template. Sampling and data collection varied across the participating countries. In the Czech Republic, while a total of 4,038 households completed a basic household module, only 1,735 women and 721 men ages 15-44 were interviewed. The four districts of Tachov, Rychnov nad Knežnou, Jeseník and Vyškovwere not included in the survey.
